Cliniko is updated, automagically.
When a form is filled in the iPad app, the patient is checked-in, their record is updated, a PDF (with signature) is uploaded to their files, and a treatment note can optionally be created as well.

Form filled
Either in-clinic, using our native iPad app, or in the patient's home, on their own Mac, PC or mobile device.

Patient updated
The patient is updated in Cliniko— name, date of birth, address, privacy policy, invoice details etc.

PDF attached
A PDF is generated — with signatures, answers and all policy text — and added to Cliniko.

Just about any of your existing forms can be converted to our format using our own purpose-built editor. Once there, they can be enhanced with visibility logic / follow-up questions based on answers to previous questions, required fields, multiple signatures (also with visibility logic) and more.
